Please use the form below to request any schedule changes. While we do our best to accommodate your request, we cannot guarantee a change will be made. Once the form is returned to your Counselor, we will meet with the individual to discuss the status of the request AFTER the first 10 days of school. There will be no changes without a change form completed and during the first 10 days of school. All request are due by Friday, September 1st.
